Getting diagnosed with AML or MDS, including CMML, can be overwhelming, but you are not alone. Taiho Oncology Patient Support is here to help, offering clear information and personal support to guide you throughout your treatment. 

With Taiho Oncology Patient Support, you can ask about:

Insurance

  • What your insurance covers
  • How you can get INQOVI through a specialty pharmacy program
  • If you have private insurance that pays for INQOVI

Costs

  • Support if you have Medicare or other public insurance
  • Assistance if your insurance does not fully pay for INQOVI
  • $0 Co-Pay Program if you have certain private insurance*

Treatment

    • Refill your INQOVI

*For commercially insured, eligible patients.

Restrictions and eligibility: Offer valid in the US, Puerto Rico, and US territories only. Only valid for patients with private insurance. Offer not valid for prescriptions reimbursed under Medicaid, a Medicare drug benefit plan, Tricare, or other federal or state programs (such as medical assistance programs). If the patient is eligible for drug benefits under any such program, this offer is not valid and the patient cannot use this offer. By presenting or accepting this benefit, patient and pharmacist agree not to submit claim for reimbursement under the above programs. Download and print the Patient Enrollment Form and fill it in at your doctor’s office with your doctor, nurse, or support staff. Fax the completed form to 1‑844‑287‑2559
